You can request one of these actions:

Refund of payments made for 1 employee/platform worker
Refund of payment made for entire submission. Requires revised wage or net income information template (CSV, 1KB)
Change of CPF Submission Number. Requires revised wage or net income information template (CSV, 1KB)
Change contributions month and year. Requires revised wage or net income information template (CSV, 1KB)
Change payment recipient (change NRIC or CPF account number)

Don't use this form for:

Correction of a previous refund or adjustment submission. Use Write to us and indicate the relevant transaction number.
Refund of payments made in excess of Additional Wage ceiling with payment dates more than one year ago. You can Write to us by selecting 'Compliance and rectifications' under Subject and 'Applying for Refund of CPF contributions' under Category.

To complete this form, you will need:

Information from the relevant Record of Payment
Revised wage or net income information template (CSV, 1KB) for refund or adjustment of an entire contributions record
