Withdrawal of savings from CPF member who has passed away

Use this form to withdraw from a late CPF member's account according to their CPF nomination

Only person(s) nominated by a late CPF member to receive their CPF savings can use this form. If you are a nominee, the CPF Board will reach out to you within 10 working days of the CPF member's passing and notify you if you need to use this form.

It is an offence to make any false statement for any purpose connected with the Central Provident Fund Act.

    Important notes

To complete the form, you will need:

  • Your CDP securities account details [if you opt for transfer of the late CPF member's discounted Singtel shares]
