Application for refund of CPF/Medisave contributions by self-employed person

Use this form to apply for refund of your MediSave contributions made to offset your MediSave payable.

 

MediSave contributions used to offset outstanding MediSave payable are not refundable unless there are excess MediSave contributions made arising from a reassessment of income by the Inland Revenue Authority of Singapore (IRAS). Applications for refund must be made within one year from the date of reassessment  .

MediSave contributions used to pre-pay future MediSave payables are not refundable unless the total amount of contributions made in a year exceeds the prevailing annual contribution limits.

 
It is an offence to make any false statement for any purpose connected with the Central Provident Fund Act.
