If you do not have any outstanding MediSave payable, you can also make a contribution that will be used to settle future outstanding MediSave payable.

Important notes:

  • If you are an undischarged bankrupt, you must obtain approval from the Official Assignee before using this form.

To complete the form, you will need:

  • Your banking app, payment app, or authentication token, if you are using PayNow.
  • Documents showing your income and expenses, if requesting an instalment plan.
