Change payer for MediShield Life, CareShield Life and ElderShield

Use this form to take over premium payer or stop premium payment for a family member's MediShield Life, CareShield Life and ElderShield policy.

If you take over as premium payer for a policy, you consent to take over payment for future premiums, as well as any outstanding premium payable.

If you stop premium payment for a policy, there will be no further deductions from your MediSave Account for that policy.

Important notes

  • This form cannot be used to change premium payment for Integrated Shield plans and CareShield Life and ElderShield Supplement policies. For such policies, please contact the private insurer directly.

To complete this form, you will need:

  • The insured person's NRIC number.
  • The NRIC numbers of the insured person's family members, if you are paying for other family member (who is not your spouse, parent, child, sibling or grandparent).
