Use this form to adjust your MediSave contributions.

 

This application form is only meant for adjustment relating to self-employed contributions.  

It is an offence to make any false statement for any purpose connected with the Central Provident Fund Act.

Important notes

  • All fields are mandatory unless otherwise indicated.
  • The request will be processed within 12 working days.

 

To complete the form, you will need:

  • A copy of the company’s BizFile, if you are a sole-proprietor;
  • A copy of the company’s BizFile / Income Tax Return Form P authorized by another partner of the company, if you are a partner in the company.
